Crises comitiales indésirables après électroconvulsivothérapie. Etude cas-témoins et revue des données de la littérature -- Electroconvulsivetherapy and seizure type side-effects. A case-control study and review of data issued from the literature

Résumé. Objectifs : Isoler des facteurs favorisant les crises comitiales indésirables (CCI) survenant au décours d’ECT et préciser la conduite à tenir vis-à-vis de la poursuite des ECT. Méthode : D’une part analyse des cas de CCI post-ECT rapportés dans la littérature entre 1946 et 1995 et d’autre part étude prospective cas-témoins sur 43 patients traités consécutivement au cours de l’année 1993 dans la même institution. Résultats : A partir des données de la littérature, les facteurs de risque de CCI sont l’existence d’une pathologie neurologique, certains médicaments, les premiers ECT d’une série et la poursuite des ECT après un premier événement comitial. Leur fréquence est mieux appréciée en cas de monitorage EEG. Dans notre étude prospective cas-témoins l’incidence des CCI est de 0,95 % des ECT; les cas présentaient plus souvent que les témoins une accumulation de facteurs particuliers (antécédents comitiaux, traitements médicamenteux, niveau d’énergie de stimulation élevé). Conclusion : Il ne semble pas possible, sur les résultats de notre étude cas-témoins et l’analyse des données issues de la littérature, d’individualiser un facteur de risque particulier des CCI post-ECT, mais l’accumulation de certains facteurs chez un patient pourrait s’accompagner d’un risque accru. La poursuite des ECT après une CCI post-ECT paraît possible sous traitement anticomitial prophylactique.

-- Summary. Objective : To determine what are the risk factors for the occurrence of unwanted seizures after an ECT session, and what is the best attitude regarding future ECT. Method : We have reviewed all the case reports published in the literature between 1946 and 1995 and have carried out a prospective case-control study of all incidental seizure type side effects among a group of 43 patients treated consecutively using the same procedure within the same institution in 1993. Results : Results from the literature are presented in tables I to IV and results from our case-control study in tables V to VIII. Twenty seven seizure type side effects are reported in 22 published papers reporting the case of 24 patients. Most accidents occur during the first ECT and the occurrence of non convulsive seizures is greatly increased in case of coexisting EEG-monitoring. In our own study the overall incidence is 0.95 %. Risk factors are neurologic disease, psychotropic medication, history of prolongued seizure during previous ECT. Conclusion : It appears that no unique risk factor but a summation of several risks (personal or family history of seizure, psychotropic medications, high energy level of electric stimuli) for specific subjects increases the risk for seizures after ECT. ECT can be resumed if necessary after occurrence of a post ECT seizure with addition of anti-epileptic medication.

Evaluation de la qualité de vie de patients psychotiques chroniques vivant en appartements protégés -- Evaluation of quality of life concerning chronically mentally ill patients living in community support systems

Résumé. Cette étude a été effectuée auprès de 46 patients psychotiques habitant dans des appartements protégés. Leur qualité de vie a été appréciée à l’aide d’un questionnaire validé, le « Profil de qualité de vie subjective » (« PQVS »). Les patients et leur infirmier référent ont chacun rempli une version identique du questionnaire. Cette étude, outre qu’elle démontre la faisabilité de telles enquêtes auprès de patients psychotiques, a un double intérêt : apporter une description, se situant en dehors de toute nosographie, du vécu du patient. Les domaines de satisfaction du patient sont relativement restreints, se limitant à l’alimentation, aux relations avec l’équipe, aux conditions matérielles de vie, et excluant la vie relationnelle ; mettre en évidence des « terrains de rencontre » ou de divergence entre les deux partenaires.

-- Summary. This study concerns 46 chronically mentally ill patients living in community support systems. Their quality of life was measured with a validated questionnaire : the « Subjective Quality of Life Questionnaire » (« SQLP »).Both patients and their care givers separately completed the same version of the questionnaire. This study first shows the feasibility of such inquiries with chronically mentally ill patients. It gives a description of the patient’s feeling which is different from the usual medical point of view. The life domains the patient is very satisfied with are mostly restricted to food, material conditions, and relationship with the care-givers, but exclude the other relationships. Our data also allow to estimate the degree of agreement and desagreement between the care-givers and their patients.

L’Unité d’Accueil et d’Orientation du CHPinel -- Psychiatric’s emergency at P. Pinel Hospital in Amiens

Résumé. Conformément aux objectifs énoncés par la circulaire de 1992 en matière d’urgences psychiatriques, le Centre Hospitalier P. Pinel s’est doté en mars 1994 d’une Unité d’Accueil et d’Orientation fonctionnant depuis son ouverture à temps partiel 15 heures sur 24. Cette structure accueille les patients en vue d’une admission en milieu psychiatrique, règle les formalités inhérentes à cette admission ou met en place d’autres solutions, après évaluation des ressources du patient et de son environnement. Elle répond également à toutes demandes de consultation psychiatrique en urgence. A moyen terme, le projet médical de l’UAO vise la permanence de l’accueil et des soins, ainsi qu’une spécialisation des prises en charge de type « centre de crise ». L’informatisation et l’analyse des données recueillies directement auprès des patients nous permettent de cerner à l’heure actuelle le profil médicosociopsychiatrique des utilisateurs de l’UAO : il s’agit en majorité d’une population masculine, âgée de 30 à 39 ans donc assez jeune, patients souvent isolés au plan familial et social. Ils sont adressés par le CHU (situé à proximité), par leur généraliste de plus en plus souvent, ou se présentent spontanément. Les pathologies les plus fréquemment observées sont les psychoses, mais des pathologies réputées plus légères, comme les pathologies névrotiques ou les troubles des conduites de type addictif, sont également très représentées.

-- Summary. According to objective expressed by 1992’s decree about psychiatric’s emergency, Philippe Pinel’s hospital in Amiens have emergency ward since march 1994. Computing activity within hospital allow to surround characteristics of patients who come to psychiatric’s hospital emergency. 2122 patients have been examined in 1995, either 6 a day. 220 were consultations, 1920 were admittance to hospital. Among these 2122 patients, 20.5 % don’t know mental hospital, 19 % don’t know psychiatry, 40 % have a relapse and 20 % where hospitalized but without met again a doctor. On the average, one hour was dedicated to patient in 1995. On a level with sociocultural, men are 59 %, which 26 % between 30 and 39 years, and 25 % between 40 and 49 years. Among the patients, 69 % live alone, and 12 % have social difficulty. Patients are sended by General Hospital (27 %), general practitioners (24 %, but this number grow in 1996), and various psychiatric institutions (10 %). The main symptoms are : sadness and depression (40 %), anxiety (37 %), sleep dysruption (31 %), alcoholism (23 %), bodilysymptoms (16 %), attempt to suicid (15 %), social problems (14 %). After observation, diagnosis are : psychosis (30 %), neurosis (23 %), alcoholism (19 %), « anti-social » personality (10 %), and drugg addiction (7 %). Among these 2122 patients in 1995, 90.5 % (1920) are sended to hospital, which 72 % with their assent, and 28 % under constraint. Consultations (9.5 %) are sent to psychiatric’s institution (43 %), general hospital (35 %), and psychiatrist (6 %). In conclusion, computing activity within psychiatric’s emergency could enable best reception, guidance, and collaboration with social or medical colleague.

Le fonctionnement de notre cerveau serait-il de nature addictive ? -- Is our brain working in an addictive way ?

Résumé. Le concept d’addiction s’impose de plus en plus comme l’émergence d’un modèle général de dépendances hétérogènes et polymorphes. Dans cet article, nous proposons d’examiner, plus en amont, ce qui pourrait fonder ce principe addictif universel, en posant la question d’une racine neurobiologique à l’addictivité. Autrement dit, le fonctionnement de notre cerveau serait-il déjà, à la base, de nature addictive? Pour pouvoir répondre affirmativement à cette question, nous regroupons deux ensembles de données : les conditions de l’addictivité cérébrale et les mécanismes cérébraux qui suggèrent un type de fonctionnement addictif. Ces données, considérées une à une, ne sont pas nouvelles, mais c’est leur synthèse en un faisceau d’indices à valeur heuristique, qu’il peut être intéressant d’examiner et de soumettre à réflexion. Le modèle que nous proposons suggère que l’architecture de notre cerveau et le type de fonctionnement de nos neurones, impliquent la présence d’une problématique addictive naturelle chez tout être humain, entre une lutte contre un bruit de fond cérébral - source de dysphorie - et un besoin de stimulations. Finalement, en reprenant certaines données neurobiologiques de ces cinq dernières années, nous proposons un modèle triangulaire du cerveau addicté mettant en jeu trois systèmes associés : le système d’éveil, le système de plaisir/déplaisir et le système de motivation. Ce modèle, en prenant appui sur celui de la recherche de sensation, va nous permettre de proposer une explication aux processus qui conduisent à l’addiction pathologique, selon les principes d’une addictivité naturelle et universelle.

-- Summary. The concept of addiction is increasingly bound to a general model of heterogeneous and polymorphous dependances. In this paper we examine upstream what could found this universal addictive principle, asking the question of the neurobiological root of addictivity. In other words, is the root of our brain working in an addictive way? To answer affirmatively to this question we gather two sets of datas : firstly, the conditions of the brain addictivity and secondly, the brain mechanisms that suggest this kind of addictive working. These datas are common if we consider them one by one, but their synthesis in a bundle of heuristic clues could be interesting to examine and to submit to thought. The model we suggest proposes that the architecture of our brain and the type of functioning of our neurones, imply the presence of a natural addictive set of problems in all human beings, between a fight against cerebral background noise - a dysphoric source - and a stimulation need. Finally, considering some neurobiological data in the last five years, we propose a triangular model of the addict brain, that involves three associated systems : « arousal » system, « liking » system and « wanting » system. This model, supported by the sensation seeking one, will allow us to suggest an explanation of the processes that lead to pathological addictions, according to the principles of a natural and universal addictivity.

Etude de validation de la version française de l’inventaire révisé de style personnel (revised Personal Style Inventory, PSI-II, Robins et al., 1994) -- Validity and reliability of the French version of the revised Personal Style Inventory-II (PSI-II, Robins et al. 1994) : study on 202 university students

Résumé. L’objet de la présente étude est de déterminer la validité et la fidélité de la version française de l’inventaire révisé de style personnel (PSI-II) de Robins et al. (1994). Le PSI-II est un questionnaire contenant 48 items répartis en deux sous-échelles évaluant la sociotropie et l’autonomie. Deux cent deux étudiants ont été inclus dans l’étude et remplissaient le PSI-II et l’inventaire de dépendance interpersonnelle (IDI) d’Hirschfeld et al. Une analyse en composantes principales sur la matrice de corrélation des 4 items suivie d’une rotation varimax a montré une structure à 3 facteurs avec 1 facteur « sociotropie » et 2 facteurs « autonomie ». Les coefficients alpha de Cronbach étaient respectivement de 0,83 et 0,72 pour les sous-échelles de sociotropie et d’autonomie. La sous-échelle de sociotropie corrélait significativement avec les 2 sous-échelles de l’IDI évaluant la dépendance affective (r = 0,67 et 0,44, p < 0,001). Les sous-échelles d’autonomie du PSI-II et de l’IDI corrélaient significativement (r = 0,39, p < 0,001). La version française du PSI-II apparaît valide et devrait permettre d’étudier les concepts de sociotropie et d’autonomie en psychopathologie.

-- Summary. The aim of the present study is to determine the validity and reliability of the French version of the PSI-II. The PSI-II is a self-rating scale divided into two subscales containing each 24 items and rating sociotropy and autonomy. 202 university students (170 females, 32 males) with a mean age of 21.54 years (sd = 4.16) were included. They filled out the PSI-II and the Interpersonal Dependency Inventory (IDI). The construct validity was explored using an exploratory principal components analysis followed by an orthogonal varimax rotation. The results had shown a three-factor solution with a « sociotropy » factor and two « autonomy » factors. The Cronbach alpha coefficients were respectively 0.83 and 0.72 for the sociotropy and autonomy subscales. The Pearson correlations between the PSI-II and IDI subscales showed significant correlation first between the sociotropy subscale and the two subscales of the IDI rating dependency, the values were respectively 0.67 (p < 0.001) and 0.44 (p < 0.001), and secondly between the autonomy subscales of the PSI-II and IDI, the value was 0.39 (p < 0.001). The sociotropy subscale items correlated significantly with the total score of the subscale with a mean of 0.46. 23 items of the autonomy subscale correlated with the total score with a mean of 0.36. The sociotropy and autonomy scores were respectively 93.98 (sd = 14.22) and 84.55 (sd = 11.69). The French version of the PSI-II had satisfactory metrological parameters and allows to study sociotropy and autonomy in French population.

Questionnaire de personnalité tridimensionnel (TPQ) : validation chez une population contrôle francophone -- Tridimensional Personality Questionnaire (TPQ) : validation of a French-speaking control population

Résumé.Parmi les échelles dimensionnelles mesurant la personnalité, le TPQ (Tridimensional Personality Questionnaire) de R. Cloninger (4, 5) occupe une place à part dans la littérature, puisque les hypothèses sur lesquelles il repose sont notamment biologiques. Ce questionnaire (100 items binaires à choix forcé) comporte trois axes : « recherche de nouveauté », « évitement du danger » et « dépendance à la récompense », tous incluant quatre sous-échelles mineures. Chacun d’eux est lié en théorie à un neuromédiateur préférentiel, respectivement la dopamine, la sérotonine et la noradrénaline. L’étude présentée ici est l’analyse de 104 volontaires témoins des deux sexes (59 hommes). Cette base de données est la première à être publiée avec une population francophone. Elle a été étudiée dans sa structure factorielle et recoupe dans une large mesure les données de la base de données contrôle de Cloninger (7). Elle paraît en conséquence adéquate pour la comparaison avec des groupes cliniques. Quatre-vingt-dix-sept pour cent des sujets présentent au moins une déviation standard sur au moins un axe et soixante-huit pour cent sur au moins deux. Ceci ne semble pas avoir été décrit auparavant et pourrait être un témoin des difficultés des êtres humains à présenter une personnalité équilibrée.

-- Summary. Among the dimensional scales that measure personality, Cloninger’s TPQ (Tridimensional Personality Questionnaire) holds a place apart in the literature, because the hypotheses it relies on are partially biological. The questionnaire (100 forced binary items) includes three axes : « Novelty Seeking », « Harm Avoidance » and « Reward Dependence », each theoretically bound to a preferential neuromediator, respectively dopamine, serotonin and norepinephrin. Each axis includes four minor subscales. The study presented here is the analysis of 104 control volunteers from both genders (59 males). This database is the first to be published with a French-speaking population. The comparison with Cloninger’s normative database shows many similarities : the mean values for the 3 axes are relatively close. The population studied here is younger and this factor could play a role in the somewhat higher mean novelty seeking scores found here. The three axes show a normal distribution. Skewness and kurtosis are between - 1 and + 1 for all the subscales, except here for NS2. Factorial structure is quite similar to Cloninger’s results. The three first axes correspond to the variables isolated in the first studies and the fourth one includes the same subscales as in the large American database. Inverse correlation between age and novelty seeking, positive correlation between female gender and harm avoidance and reward dependence were also found. However, no correlation was found between novelty seeking and male gender. A correlation was found between level of instruction and NS1 only, not with the whole NS axis. Eighty-seven% of volunteers presented with at least one standard deviation on at least one axis and 68% on at least two. This does not seem to have been described previously. It can be a sign of the difficulty of human beings to show a balanced personality. In conclusion, the database presented here shows many similarities with Cloninger’s normative database. This underscores its value for comparisons in clinical trials in the future.

Boulimie et auto-immunité -- Bulimia nervosa and auto-immunity

Résumé. Cette étude a porté sur 31 patientes boulimiques (critères DSM III R). Elle a visé à évaluer le statut auto-immunitaire de ces patientes. Les auto-anticorps étudiés réagissent vis-à-vis d’un panel antigénique de neurotransmetteurs cérébraux : dopamine, dopamine ß-hydroxylase, et sérotonine. Nous avons mis en évidence une baisse significative des auto-anticorps dirigés contre ces antigènes cérébraux dans la population étudiée comparativement à la population témoin appariée pour l’âge et le sexe. Cette baisse des auto-anticorps naturels pourrait être impliquée dans un phénomène étiopathogénique lié au système sérotoninergique.

-- Summary. In the first part of this study, we investigated the rate of natural autoantibodies, in a sample of 31 female inpatients with bulimia nervosa according to DSM III-R criteria. The control (age and sex matched) group consisted in high school students including 10 females without eating disorders, depressive disorder or immunological disease. We investigated especially natural autoantibodies reacting with compounds of the central nervous system (Dopamine, Dopamine ß Hydroxylase, Serotonine). Our first conclusion is that there is a lower level of these natural auto-antibodies among female patients with bulimia nervosa. In the second part of the study, we have especially investigated the correlation between impulsivity in bulimia nervosa and the rate of natural autoantibodies against serotonine.

Absence de dysfonctionnement frontal et de trouble de la mémoire de travail chez de jeunes schizophrènes -- Absence of frontal lobe dysfunction and working memory deficit in young patients with schizophrenia of recent onset

Résumé. De mauvaises performances dans les tests sensibles au dysfonctionnement frontal ont été retrouvées de façon répétée chez les schizophrènes. Cependant, il n’est pas clairement établi si ce dysfonctionnement est partie intégrante de la maladie, ou s’il résulte de facteurs confondants tels que l’ancienneté d’évolution, le retrait social, ou les médications antidopaminergiques. Nous avons donc étudié les performances de 12 adultes jeunes et actifs, atteints de schizophrénie chronique-résiduelle et de 12 sujets témoins, à des tests de la lignée frontale, et à une batterie de tests mise au point pour tester la mémoire de travail, dont certaines composantes sont sous la dépendance du cortex préfrontal. Nos résultats montrent que les performances des schizophrènes sont comparables à celles des témoins pour la plupart des tâches, en particulier celles testant la composante centrale de la mémoire de travail. Le faible nombre de sujets ne permet pas de conclusions définitives, mais cette étude suggère que le dysfonctionnement frontal ne semble pas être présent précocement dans la schizophrénie, chez des patients socialisés et actifs.

-- Summary. The impairment in tasks requiring intact frontal lobe functions has been repeatedly shown in schizophrenics. However, the relative roles of confounding factors, like duration of the disease, social withdrawal, or antidopaminergic medication, are not clearly demonstrated. We studied the performance of 12 young active patients, with chronic residual schizophrenia that had recent onset, and 12 control subjects, with frontal lobe tests and with a battery designed to explore working memory. The results show normal performance in schizophrenics. The small number of patients does not allow definitive conclusions, but this study suggests that a frontal dysfunction may not be present early in the evolution of schizophrenia in active patients.

Résumé. Cette étude clinique a consisté à étudier une population de 32 patients mélancoliques traités par le chlorhydrate de fluoxétine à dose fixe (20 mg/j). L’examen clinique avec évaluation de l’effet antidépresseur a été effectué sur l’échelle HDS/MES et les patients ont été répartis en trois groupes (répondeurs, répondeurs partiels avec ou sans rechute, non répondeurs) selon l’évolution clinique des malades en cours de protocole. Un dosage systématique de fluoxétine et norfluoxétine a été réalisé lors de la troisième semaine. Dans notre étude, 53% des patients ont réagi positivement après 21 jours de traitement. Notre travail n’a pas montré de corrélation entre les scores psychiatriques et les concentrations plasmatiques en fluoxétine et norfluoxétine.

-- Summary. The aim of this clinical study was to investigate 32 melancholic patients treated by fluoxetine (20 mg/day). The clinical examination to evaluate the antidepressant effect of fluoxetine was realized by using the HDS/MES criteria. The patients were divided into three groups (responders, partial responders with or without a relapse, non responders) according to their clinical evolution during treatment. Fluoxetine and norfluoxetine were evaluated by HPLC after 3 weeks of treatment. In the present study, 53% of the patients have a positively reaction to the 21 day’s treatment. Our results showed no correlation between the psychiatric scores and the plasma concentrations of fluoxetine and norfluoxetine.

Résumé. Nous rapportons deux cas de patients ayant présenté une éruption cutanée après prescription successive d’antidépresseurs inhibiteurs sélectifs de la recapture de la sérotonine. Différentes molécules sont impliquées : fluoxétine, paroxétine, fluvoxamine, sertraline. Elles ont été prescrites chez ces deux patients dépressifs successivement, en respectant des intervalles libres suffisants. Des réactions cutanées évoquant une toxidermie sont apparues avec chaque molécule. La chronologie est fortement évocatrice de leur imputabilité. Il n’existe un excipient commun que pour trois de ces molécules. La question d’une allergie croisée se pose, question d’autant plus intéressante que les structures chimiques de ces molécules sont très différentes.

-- Summary. Toxidermia is a well-known complication of antidepressant therapy. The authors report two cases of adverse cutaneous reactions after treatment by several serotonin uptake blockers. The first case concerns a 38-year-old woman, who suffered from depressive disorder. She has been treated with fluoxetine. After eight days, a cutaneous reaction appeared. This reaction subsided when fluoxetine was stopped. Two months later, she had a prescription of paroxetine, and the same skin adverse effect appeared. The cutaneous lesion deacreased with the suppression of paroxetine. The chronology of the eruption suggests that it was caused by an adverse reaction to the both serotonin uptake blockers. The second case concerns a 40-year-old man who had several prescriptions of antidepressant treatment in a period of one year : fluoxetine, fluvoxamine, paroxetine and sertraline. Each time a cutaneous reaction appeared. It is advisable to substitute after an adverse effect a medication from one of the other classes of antidepressants. But what we saw in these cases is that the serotonin uptake blockers could be involved in the same allergic reaction while these drugs have different chemistry structures. Only three of them have the same excipient. These cases ask the question of cross-reactivity between the serotonin uptake blockers.

Manifestations psychiatriques comme seul signe clinique d’hypothyroïdie. A propos d’un cas -- Psychiatric symptoms as single manifestation of hypothyroidism

Résumé. L’hypothyroïdie se manifeste le plus souvent par l’association de troubles somatiques bien connus et de troubles psychiques dominés par des symptômes dépressifs et/ou un affaiblissement intellectuel. Mais les symptômes psychiatriques peuvent être très variables et les signes somatiques discrets, voire absents. Nous rapportons l’observation d’une homme de 63 ans dont l’hypothyroïdie n’a été diagnostiquée qu’après 6 mois d’évolution du fait de sa symptomatologie dominée par une agitation anxieuse et un vécu persécutif, sans signes somatiques évocateurs, d’une amélioration transitoire sous neuroleptiqueet d’un TDM mettant en évidence une légère atrophie corticale. Le traitement étiologique a amené la disparition des troubles de l’humeur et du comportement. Quelques troubles cognitifs persistent comme c’est souvent le cas lorsque le traitement a été instauré après plusieurs mois d’évolution. Cette observation rappelle la nécessité systématique de pratiquer, chez tout patient présentant un syndrome psychiatrique d’apparition récente, un bilan complet explorant notamment la fonction thyroïdienne.

-- Summary. This article intends to demonstrate, by presenting a clinical case, the importance of the thyroid examination in psychiatric practice. After a brief presentation of the evolution of the conceptions regarding the relation between thyroid insufficiency and psychiatric pathology, a clinical case is reported. This 63 years old man was admitted in a psychiatric ward because of his persecutive delusions and behavioral disorders (agitation, agressivity). Cognitive (namely memory) impairment was also present at the time of admission. The endocrine investigation has documented, in spite of the absence of any suggestive physical signs, a thyroid insufficiency. After the replacement treatment, all symptoms but the cognitive dysfunction disappeared. The patient’s evolution is presented clinically, also rated on MMSE and MADRS scales, and biologically (TSH and T4 determination) for a 4 months period. The absence of any pathognomonical psychiatric finding, the possibility of the absence of other signs and symptoms (namely physical) in the hypothyroid state, the presence of potentially irreversible cognitive deterioration, as well as the inocuity and sensibility of thyroid hormones examination justify the systematic thyroid evaluation for all new psychiatric patients.
